Job Title: Care Coordinator

Job Description:
MetroHealth HMO is seeking a dedicated and compassionate Care Coordinator to join our team in Lagos. The successful candidate will be responsible for coordinating patient care, liaising with healthcare providers, and ensuring seamless service delivery to our enrollees.
Key Responsibilities:
Coordinate and monitor patient care plans
Serve as a liaison between enrollees, healthcare providers, and internal teams
Ensure timely authorization, follow-up, and continuity of care services
Maintain accurate patient records and prepare reports
Support quality improvement initiatives and enhance customer satisfaction
Requirements:
Must be a Medical Doctor
Minimum of two (2) years post-NYSC work experience
HMO experience is desirable
Strong communication and interpersonal skills
Good organizational and coordination abilities
Proficiency in basic computer applications
Experience in a healthcare or managed care environment is an added advantage
