Job Description

We are seeking a licensed Physical Therapist (PT or DPT) to provide skilled, patient-centered care in private residences. Responsibilities include evaluating functional status, developing and implementing individualized treatment plans, improving mobility, managing pain, preventing falls, and educating patients and families. Candidates should possess a current PT license, strong clinical and communication skills, a valid driver’s license, and the ability to work independently; home health experience is preferred.
